Who can receive DVA Community Nursing Services?
The DVA Community Nursing (CN) Program funds clinically required nursing and personal care services provided in your home by qualified nurses and support staff. Services can include help with medication, wound care, hygiene/ showering, dressing and palliative care.
You are eligible for an assessment if you have:
Check with your GP - they can send a referral for services directly to Care to Care!

The Veterans’ Home Care (VHC) Program provides practical help at home so you can continue to live independently.
You are eligible for an assessment if you have:
For Veterans' Home Care Services, call a VHC Assessment Agency directly on 1300 555 450 to have an assessment for Services.
Once you have been approved, please request Care to Care as your provider.
